Consent to Participate in WEB Youth (2021-2022)

Understanding that the purpose of WEB Youth Ministry is to provide opportunities for kids to engage together in active recreational pursuits, social activities, and Christian education, I acknowledge the following conditions of enrollment:

The parents/guardians submitting this application must have legal custody to do so. Conditions of custody, if applicable, must be fully communicated to West Edmonton Baptist Church.

I acknowledge that participating youth are expected display reasonable behavior that ensures safety, respect and consideration of others.

I agree to review all rules, guidelines and instructions as well as discuss appropriate behavior and consequences with my youth. I understand that if my youth is dismissed it is my responsibility to arrange a way home immediately.

I acknowledge that WEB is not responsible for lost, damaged, or stolen personal property brought to the program. I shall assume financial responsibility for my youth’s actions which causes damage to the property of others.

In the event of an emergency, I give the designated leader of the WEB Youth Ministry permission to obtain any medical treatment for my youth and will cover the full cost of an ambulance and any other costs entailed.

I understand that during WEB Youth Ministry programming time Christian educational and experiential practices such as Bible studies, Scripture verses, Christian songs, prayer times, and discussion will be integrated in a child-friendly, non-coercive manner. Children from any faith background are welcome to attend, however Christian values and beliefs will be presented.

I give West Edmonton Baptist Church permission to take photos or videos (digital or otherwise) of myself, my child(ren)/family members for use in educational materials, publications, promotional materials, the West Edmonton Baptist Church website, Facebook and other social media sites and/or other materials and release West Edmonton Baptist Church and all persons acting under its authority from any claims I might have due to the initial or subsequent publication of such materials. Participant’s names will not be used.

Electronic communication through avenues such as texting and social media is an important part of life and relationships in today's youth culture. It is not uncommon for youth to reach out to youth leaders using these technologies (e.g. friending on Facebook, joining the WEB Youth Facebook page, communicating via Instagram, Twitter, text message and/or email, etc). Furthermore, this style of communication is integral in keeping youth connected and keeping information current. This contact will be for the purpose of mentoring, sending out program reminders, announcements, cancellations, gauging interest in an activity, and/or daily devotions/inspirations. Records of this contact will be kept. I give permission for the WEB Youth Leaders to respond to or initiate communication with my youth through the following means: Social Media, Texts, Emails, etc.

Parents/Guardians, please read the following carefully with your youth.

1. All youth attending WEB Youth must have a registration completed and submitted to the West Edmonton Baptist Church Office. All parents/legal guardians must be listed.

2. Youth must be in Grade 7—12 in order to attend WEB Youth.

3. Please discuss appropriate behavior and the consequences of inappropriate behavior with your youth. The following behaviors are expected and if not demonstrated will be addressed with consequences which may include: sitting out from an activity, being sent home, temporary or permanent suspension.
*Respect leaders, other youth, and property
*Listen to and obey the leaders
*Zero tolerance for violence, bullying and discrimination
*No foul language
*No use of tobacco products, non-prescription drugs/illegal drugs, or alcohol during WEB Youth times

4. DRESS CODE: Respect the WEB Youth dress code. The 5-B Rule must be followed. No bosoms, butts, bellies, bras or boxers can be shown. All shorts and skirts should reach mid-thigh and all bathing suits should be one piece or tankini style for girls and boxer style shorts for boys.

5. Electronics such as cell phones, iPods, and tablets will not be allowed during WEB Youth programming time.

I hereby acknowledge that I have voluntarily registered my child in a Children’s Ministry of West Edmonton Baptist Church. I am aware of the risks that may arise out of participation in the activities being offered including but not limited to gym games, crafts, and sporting activities. I agree on my behalf and on the behalf of my family that West Edmonton Baptist Church shall not be liable for any personal injury, death, or property loss, and I indemnify West Edmonton Baptist Church from any such claims for negligence, or breach of statutory duty of care on the part of West Edmonton Baptist Church or its directors, officers, employees, or volunteers, and I waive the right to make any claim with respect thereto.
